Due April 15:
Registration Form (Junior and Senior High)
Interest Group Form (Senior High Only)
$50 per person transferable, non-refundable deposit
Make payable to Junior High or Senior High Lakeside
Please do not send one check for both camps.

Due May 15:
Emergency Medical Form
Copies of applicable insurance cards
Final Payment

So You Wanna Be On Stage?

Please don’t forget about Lakeside housing as an
option for Senior High Lakeside!  You can request any
of these housing options for the
$325 per person weekly rate.

 WoHoMis is great for groups of six or less
 Hill Top can house a group of thirty or less
 The Annex can house a group of twenty or less
 The Cabins give private accommodations for groups
of six to twelve.
 The Erie Guest House (2nd week only) will house
up to twenty-one.

$100 “Unchurched” Youth Credit
For every “unchurched” youth your group brings to Lakeside
2009, you will receive a $100 credit voucher*
for Lakeside 2010.
*Credit voucher is subject to camps meeting minimum occupancy requirements.

This is the beginning of something larger than even we can
appreciate.  We have been given the opportunity to double
the amount of people we can expose to what we know is an
amazing Christ-centered experience.  Please prayerfully
